Pigs may only be registered by the Breeder.
Fill in Litter Number – you may start with any number, but may not use that number more than once
Date of Birth – Farrow Date.
Total Pigs Born – Number of pigs farrowed by the sow
Total Born Alive – Number of pigs born alive to the sow
Total Pigs Raised – Number of pigs raised to weaning.
Select your registration payment – you may apply/renew membership along with litter registration; please enclose additional annual membership fee.
Owner – please list your Name, Farm Name (if applicable), Mailing address and a contact phone number in case something is not clear with your litter registration application.
Breeder – please list your Name, Farm Name (if applicable), Mailing address and a contact phone number in case something is not clear with your litter registration application.
Pedigree Information: Names and Registration numbers of the SIRE and DAM MUST be listed on the application. Grand-Sire and Grand – Dam is required to be listed. If Grand-Sire and Grand-Dam is not known, please attach a detailed explanation of where and when the Sire and Dam were acquired by the Owner or Breeder.
Service Certificate for Bred Sow Only – If you are not the owner of the Boar; have the Owner of the Service Boar fill out the certification and sign.
Individual Pigs in Litter – all pigs must be listed individually: ALL BOARS AND GILTS MUST BE PERMANENTLY IDENTIFIED – EAR NOTCHING, TATTOO OR PERMANENT EAR TAGS ARE ALL ACCEPTABLE METHODS OF PERMANENT IDENTIFICATION.
Two photos of each pig must accompany the litter registration application. It is best if the pig is standing by itself, not held: The right side of the pig: full length of the pig, photo of the right side and the left side of the pig: full length of the pig, photo of the left side
Please identify each photo of each pig with the following: Farm name or initials, pig name, ID # (Litter – Count): for example: LFF Princess 1-1 right.jpg. Remember to send an image file, do not send PDF.
